Care Logistics


Aim 4 Excellence is a care coordination model that introduces Care Logistics, a new system designed to improve patient flow throughout the hospital system. A cornerstone of the project is new clinical technology software that monitors the status of patients from the time they are admitted to the hospital, through the scheduling of tests and treatments, to preparation for discharge.



Our Goal is to Increase Efficiency and Effectiveness, Decrease Costs and Improve Patient Care.

Aim 4 Excellence will:

  • Contain the implementation of two systems for Patent Logistics and Order Logistics.
  • Redesign core processes that involve admission, scheduling, transport and discharge preparation, among others.
  • Introduce a new role, Manager of Care Coordination(MCC) to expedite care processes and patient flow.
